Total Parenteral Nutrition (TPN)

What is TPN?

Total parenteral nutrition (TPN) supplies all daily nutritional requirements. TPN can be used in the hospital or at home. Because TPN solutions are concentrated and can cause thrombosis of peripheral veins, a central venous catheter is usually required. Plaza Home Care Pharmacy delivers high quality infusion management and personalized care for patients on TPN therapy. The goal is to provide cost-effective delivery of this complex drug therapy that ultimately reduces hospital stays. Our TPN program maintains the patient in a clinically stable condition throughout their treatment tailored to their specific diagnosis and nutritional needs.

Through clinical oversight of the patient's condition, we can assist with the prevention of potential complications to produce positive therapy outcomes.

TPN Program Components:

  • Identification and Assessment of Needs

  • Evidence-Based Clinical Protocols

  • Collaborative Practice Models

  • Patient Self-Management Protocols

  • Ongoing Process Assessment Resulting in Improved Outcomes

  • Ongoing Collaborative Communication Between Patient, Physician, Health Plan and Ancillary Providers
